Your cancer care team needs to know about your symptoms and other concerns so that they can help you maintain and improve your quality of life. You can tell your cancer care team about your symptoms by completing a questionnaire at a kiosk called the Your Symptoms Matter. You should complete the questionnaire at every clinic appointment.
Once you have checked in with registration your next step is to visit a kiosk or borrow a table to complete the Your Symptoms Matter questionnaire
The kiosks are located in the Radiation Oncology waiting area (Burr 0) and in the Cancer Clinic waiting area (Burr 1). There are also several tablets available for you to use which you can find in the Cancer Clinic waiting room (Burr 1).
To start the questionnaire all you need to do is swipe or manually type your health card. The questionnaire asks you to rate 9 symptoms that are common to cancer patients on a scale of 0 to 10. A 0 means the symptom is absent and 10 means it is the worst possible.The questionnaire takes less than five minutes to complete.
* Head and neck cancer patients will have additional questions on their Your Symptoms Matter questionnaire and are asked to arrive to their appointment time 10 minutes earlier to complete the questionnaire
